Successful Intervention Strategies

As exam results become more analysed, and teachers and departments more accountable, how are whole schools and individual departments managing intervention. Especially now that so many subjects (history included) have moved to modular exams. This forum is to share the intervention strategies and ideas that have been successful.

I have attached a form which we use to begin the intervention process. This is just a history dept thing, not a whole school thing. It is filled in after discussion with the students when the first assessment it done. It should then be revisited after each assessment. Students take it home to share with their parents.
